PROGRAM COORDINATOR (FULL-TIME)

Location:
Coordinate outreach and testing activities and implement assessment and linkage of high risk individuals to appropriate substance abuse treatment and HIV/AIDS services.
Participates in the following: Activities of the Substance Abuse Capacity Expansion Project; Provide grant activities throughout targeted area(s) and assure that grant deliverables are met by program team; Implementation of approved strategies to effectively accomplish objectives for street outreach, substance abuse pre-treatment screening, and HIV antibody rapid testing in designated areas with the targeted population.
The selected candidates will also implement approved strategies to effectively refer and enroll individuals at need and eligible for substance abuse treatment. Responsible to monitor the enrollment of referred clients in services of substance abuse treatment. Provide assessment for Substance Abuse Treatment among the target population. Responsible to provide substance abuse evaluations, discharge interviews, and six months follow up interview with clients. Coordinate outreach activities following CAP protocols related to outreach, safety, referrals and linkages. Administer pre/post tests and/or other survey tools to evaluate effect of materials and strategies as requested by the Program Supervisor
Position require: Bachelor’s degree in Social Work, Health, Behavioral Sciences or Education services; Possess experience related to delivery of services in a social service setting; Experience providing services to individuals with chemical dependency; Current knowledge of HIV/STD/TB, disease transmission and HIV/AIDS and substance abuse prevention strategies; Knowledge on the dynamics of a diverse population, including verbal and written skills adaptable to diverse audiences; PC Proficient (Word, Excel, Outlook); Willingness to work flexible schedule, including evenings or weekends; Valid Florida driver's license, automobile insurance and reliable automobile; Ability to transport self to various community locations throughout the county where outreach activities take place. Preference for experience in HIV/AIDS, counseling and chemical dependency; Completion of AIDS 101, 104 and 501 certification in IV/counseling/testing Salary: $29 - $31K / year, CAP offers an excellent benefits package plus a retirement program. Positions are part-time, 15 hours per week. PAL’s will conduct face-to-face individual sessions with clients accessing CAP services to explain the various services and the eligibility criteria. Each PAL will have an assigned caseload (averaging 20-25 clients) for ongoing treatment education and adherence services. PALs will co-facilitate educational/support groups and/or Chronic Disease Self-Management groups for people living with HIV/AIDS.
