FoundCare HUD INSPECTION SERVICES RFP

DESCRIPTION OF SERVICES

The purpose of this solicitation is to provide Comprehensive Community Care Network, Inc. dba FoundCare with HUD Inspection services using HUD Form HUD-52580 at a competitive rate for the grant year term from 10/1/2010 through 9/30/2011 and any renewal periods agreed upon. FoundCare is contracted with HUD to provide housing that is safe, decent, and sanitary, (per the regulations at 24 CFR 574.310(b) for our clients throughout Palm Beach County. The HUD Inspector will be responsible for completing Annual inspections, some re-inspections and some exit inspections (approximately 208).

 

Bidders should read the entire set of specifications carefully, as these will form the basis of the contractual Agreement with FoundCare. Failure to comply with the specifications may provide grounds to nullify the Contract Agreement. Bids should reflect only the expected cost to FoundCare associated with HUD Inspections congruent to the specifications listed herein.

 

REQUIREMENTS

Qualifications

During the period of this Agreement, candidate agrees to perform the duties pursuant to this Agreement that meet the following qualifications:

(a) All applicants must be experienced in performing HUD Inspection services (Form HUD-52580)

(b) All applicants must be capable of covering the entire Palm Beach County.

PROGRAM SUPERVISOR

(HIV Prevention Department)

The Comprehensive AIDS Program (CAP) of Palm Beach County seeks an individual to deliver activities which includes, but not limited to: Supervise a group of 6+ individuals. Supervise, coordinate and deliver HIV prevention activities targeting individuals at very high risk for HIV in Palm Beach County. Supervise the activities of HIV Testing and outreach. Responsible for the implementation and development of several HIV prevention interventions including RAPP, RESPECT and HIV Counseling Testing and Referral. Conduct direct observation of staff in field for quality assurance purposes. Provide direct feedback to staff on performance and ways to improve program and reach program objectives. Implements and assures that program adheres to agency policy and procedures.

 Position Requires: A professional individual who must be knowledgeable and familiar with local base Palm Beach County Social Services, including service organizations serving persons with HIV-spectrum disease and their families; Bachelor’s degree in Social Work, Health, Behavioral Sciences preferred  or a related field;  3 years of supervisory experience; Current knowledge of HIV/STD/TB disease transmission and prevention strategies, completion of HIV/AIDS 104 and HIV/AIDS 501 courses a plus; Willingness to work flexible schedule including evenings and weekends;  PC proficient possessing the ability to use internet and create spreadsheets, reports, charts and brochures; Ability to communicate with clients, families and staff from community agencies and engender cooperation from same; Valid driver’s license, automobile insurance and reliable automobile; maintain confidentially.

 Salary: $31K - $34K / year, plus CAP offers an excellent benefits package including a retirement program.

PATIENT ADVOCATE

Comprehensive AIDS Program (CAP) of Palm Beach County, Inc. is seeking a dynamic, dedicated, professional individual who can provide compassionate services to fulfill the role of Patient Advocate. As Patient Advocate you’ll be responsible for:

 

Link people living with HIV/AIDS to specialty medical care and other services as indicated.  This includes assisting patients with accessing patient assistance programs for medications. (estimated 25% of time).

Coordinate and provide treatment education services for people living with HIV/AIDS throughout Palm Beach County. (estimated 50% of time).

Conduct outreach activities to enroll families with children in Florida’s health insurance programs. (estimated 25% of time).

 

Position requires a Bachelor’s degree in Social Work (BSW), Human Services, Psychology, Sociology, or a related field; Professional individual who is knowledgeable and familiar with local base Palm Beach County Social Services; must be able to effectively manage their time, be self-motivated, computer literate, and able to accurately document their activities in the community.

CAP offers a competitive salary ($28,000-$30,000, DOE) and an excellent benefits package plus as well as a retirement program.
