Hours
FoundCare Health Center is seeing patients Monday - Friday from 8:00 a.m. - 5:00 p.m. with extended hours every other Thursday until 7:00 p.m. Closed from 12:00 – 1:00 p.m. for lunch.
Once fully funded, the health center will offer appointments through the lunch hour as well as additional evening hours and Saturday morning clinics. Dental and pharmacy services are coming soon.

Conveniently situated on a Palm Tran bus route, FoundCare Health Center is easily accessed between Forest Hill Boulevard and 10th Avenue North, on the east side of South Congress Avenue.
Directions from I-95 (North or South)
Exit at Forest Hill Blvd. and travel West
Proceed to the intersection of Forest Hill Blvd. and Congress Avenue
Turn left onto Congress Avenue heading South
Turn left at the second traffic light at Dolan and enter the parking lot
Make an immediate right turn and go through the stop sign, our building is on left and parking is to the South
Please enter the building on the South side through the Lerner Atrium
