Web-Based Assessment
An on staff FoundCare Patient Eligibility Navigator will meet in a private setting to assess your individual and family eligibility using web-based technology for standardized assessment throughout Palm Beach County. Both walk-in and scheduled appointments are brief – no long forms to fill out. Information is entered into our system and based on income we can determine what programs you may be eligible for: Medicaid, Medicare, Health Care District Option 1, Healthy Palm Beaches, VITAS.
 

Please bring to your appointment:

  • Photo Identification
  • Social Security Number
  • Household income documents (pay stub, unemployment form, previous year’s tax form, names and birthdates of all members of household, address, contact phone numbers.)
  • Insurance Card
  • Past medical history or records

Applications completed online
FoundCare Health Center’s Eligibility Patient Navigator will assist you with Medicaid, Medicare, Florida KidCare, Health Care District Option 1, Healthy Palm Beaches, VITAS and other online applications. We can provide you with online access to complete applications on your own. Your eligibility assessment will transfer automatically into FoundCare Health Center’s secure online applications. Sliding Fee Scale
FoundCare Health Center provides quality healthcare services regardless of patient’s ability to pay. If uninsured, a trained Eligibility Patient Navigator will assess each patient’s household income to determine if you are eligible for the sliding fee scale established by the FoundCare Board of Directors. If you are eligible, you will pay only a small fraction of the cost of the services provided. If you are eligible for a 100% discount, a minimum fee of $20.00 is requested.

FoundCare Health Center requests a minimum fee of $20.00 to be paid at the time of service for patients without insurance. If you are unable to pay at that time the fee will be waived and you will still see your provider. You will be assessed for the type of payment you are eligible for each time you have an appointment.

FoundCare Health Center accepts payment by cash, credit/debit card, and checks.
 

Referrals and Specialty Care
For patients needing additional specialty consultation or diagnostic services, FoundCare Health Center participates in Project Access. Center staff will coordinate communication of your referral to the qualified staff member at Project Access to locate participating local specialists. These services are provided at low or no cost.

For example, patients requiring mammography are referred to Project Hope which provides convenient mobile mammography at the health center location and a variety of locations throughout Palm Beach County. Check the events calendar for upcoming dates at the health center.

Referrals are made only for FoundCare Health Center patients who have seen a provider and determined that a referral is necessary based on established clinical findings.
 

Prescription Assistance Programs
FoundCare Health Center’s policy is to prescribe generic or lowest cost medications that meet treatment standards. Prescriptions are sent electronically to your local pharmacy whenever possible for patient convenience. FoundCare Health Center prescribes medications covered under reduced cost programs through WalMart, Publix, Walgreens and other participating pharmacies.

When generics or special pricing is not available, health center staff will help patients apply for Prescription Assistance Programs offered by most major pharmaceutical programs. These programs are often income based for eligibility and may include medications, supplements and supplies needed to maintain your health. If you find that you cannot afford the cost of medications prescribed and recommended by your provider, please consult our Eligibility Patient Navigator at Eligibility@foundcare.org to complete the required applications. Participating companies will send your medications directly to your home, to the health center, or by other arrangements. Center staff will often provide sample medications, if available, until your request is filled by the Prescription Assistance Program.

A prescription is required from a FoundCare Health Center provider in each case and you must be an established patient to participate.
