FoundCare Health Center Providers
The nonprofit FoundCare Health Center is under the direction of a Center Manager and Medical Director, and is staffed with two Advanced Registered Nurse Practitioners (one for Family Practice and one for Pediatrics), one Pediatrician and one part time Infectious Disease doctor. Administrative staff includes a Nurse Manager and Front Desk Manager. Clerical and nursing staff are cross-trained to assist one another during this start-up timeframe.
 WILLIAM D. QUINN, Center Manager
Bill Quinn serves as FoundCare Health Center's Center Manager. He is in charge of overseeing the daily activities of the Center, including staffing, grant writing, procurement, patient flow and billing practices. Even prior to joining the health center, Bill committed his career to serving communities that are traditionally underserved for health and social services. He served as the first Ryan White Title II Coordinator for the State of Wisconsin; created and implemented the first RW Title III early intervention clinic in urban Milwaukee, Wisconsin; co-founded an integrated comprehensive behavioral and primary health center for families in an urban African American community; and developed services in Hispanic, Latino and Marshall Islander communities to provide accessible and culturally relevant healthcare services. Bill has a Masters of Science in Social Work from the University of Wisconsin-Madison.
 LESLIE E. DIAZ, M.D., Medical Director
Leslie E. Diaz, M.D. serves as FoundCare Health Center's Medical Director. She is responsible for the overall medical supervision, leadership and provision of direct customer care within the Center. In practice since 1996, Dr. Diaz is board certified and received her M.D. at Universidad Central Del Este in the Dominican Republic and completed her residency at Northeast Ohio University College of Medicine in 1994. She received a fellowship from the University of Florida with a specialty in infectious disease in 1996. She has been honored by Miami magazine as one of "100 Best Physicians in South Florida," as "Doctor of the Year" by Columbia Hospital, and as "Attending of the Year," by Nova University's Medical Education Program.
 JOHANNA ALBRECHT, M.D., Pediatrician
Pediatrician Johanna Albrecht, M.D. is responsible for planning and carrying out the medical care services for children from birth through transition to adulthood at FoundCare Health Center. Dr.Albrecht is board certified in pediatrics and received both her medical and bachelor's degrees from Cornell University. An active community member and advocate, Dr. Albrecht has served as a member of Delray Beach's Full Service School Headstart Program Medical Advisory Committee, a member of Florida's Health/Education Consortium, a member of the South Florida AIDS Consortium, and a nominee for St. Lucie County's Child Advocate of the Year Award. Multi-lingual, Dr. Albrecht speaks Spanish and some French.
 ELSIE REMY, Advanced Registered Nurse Practitioner
Elsie Remy serves as FoundCare Health Center's Advanced Registered Nurse Practitioner. In this position, Ms. Remy is responsible for conducting complete physicals, providing treatment and counseling patients. Ms. Remy was most recently an Advanced Registered Nurse Practitioner at Resolution Medical Services, Inc. in West Palm Beach. She has extensive experience in intensive care, acute and primary care units. She received her BSN from Long Island University in Brooklyn, NY; Master of Science in Adult Health Nursing and Post Masters in Family Practice at Stony Brook University in Stony Brook, NY. She is nationally Board certified. Remy is a member of The American Academy of Nurse Practitioners, American College of Nurse Practitioners and Florida State Nurses Association.
