Schedule an Appointment
FoundCare Health Center serves patients regardless of their ability to pay. Eligibility specialists meet with each patient prior to their first appointment to establish a portion of responsibility for the bill, based on household income and using the federal poverty level. FoundCare Health Center is not a "free clinic." Staff work individually with each patient and family to make healthcare affordable. Initially, the Center will accept Medicaid, Medicare, and Palm Beach County Health Care District programs and the related HMO’s. In the long term, FoundCare Health Center will accept other commercial insurances. All patients receive the same high-quality standard of care.
What to bring to your appointment:
- Photo Identification
- Social Security Number
- Household income documents (pay stub, unemployment form, previous year’s tax form, names and birthdates of all members of household, address, contact phone numbers)
- Insurance Card
- Past medical history or records
