FoundCare is committed to providing your family with the best medical care possible by offering a wide range of services. These services include pediatric and adult primary care, chronic disease management, behavioral health services, women's health services, dentistry, laboratory services, X-rays, and an on-site pharmacy. A variety of our services are accessible throughout our centers in Palm Beach County, with our West Palm Beach center conveniently offering all of our services in one location.

Our culturally competent staff is fluent in English, Spanish, Haitian Creole, and French. Office hours vary by location and include daytime, evening, and Saturday appointments.

If your health plan is not listed, our team of eligibility specialists will work with you and your insurance company to establish FoundCare as your medical home.

Our goal as a community health center is to provide comprehensive quality health care to our patients. Every patient seen by FoundCare providers receives the same high-quality standard of care regardless of whether or not they have insurance. We offer a sliding fee scale based on family size and income for those who are not covered by insurance. You may ask us to conduct a financial assessment to determine your eligibility for our sliding fee scale. No one is turned away for inability to pay.

Please have the following items with you when you arrive to your scheduled appointment:

  1. Photo ID (all patients)

  2. Uninsured patients, please provide income verification (pay stub, W2, or tax return)*

  3. Unemployed patients, please provide verification of Unemployment (SEQY) or Disability (TPQY) from Social Security

  4. Undocumented and uninsured patients must provide a Letter of Support (Valid To/From Dates, the contact information of supporter, and be notarized)*

  5. Insurance card (if applicable)

  6. Past medical history or records

  7. Registration Consent Form. Copies will be provided at the time of appointment if you need assistance filling out or do not have access to a printer.
